INFORMED CONSENT

I have provided accurate and complete information regarding my health. I understand that sports massage and soft tissue therapy involve physical manipulation of muscles and tissues.

I understand that treatment may result in temporary soreness, redness, or mild bruising. I understand that I may withdraw consent and stop treatment at any time.

I confirm I am over the age of 18. If under 18, a parent or legal guardian must be present during the treatment session and must sign below.


 

GDPR & DATA PROTECTION CONSENT

McNally Performance & Recovery complies with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

Your personal and health data is collected solely for the purpose of providing safe and effective treatment. Data will be stored securely and retained for 7 years following your last treatment (or until age 25 for minors).

You have the right to request access to your data, request corrections, or request deletion where legally permissible. You may also contact the Information Commissioner’s Office (ICO) if you have concerns.

 

CANCELLATION & LATE POLICY

A minimum of 24 hours' notice is required to cancel or reschedule an appointment.
Cancellations made within 24 hours will be charged at 50% of the treatment fee.
Failure to attend without notice (no‑show) will be charged at 100% of the treatment fee.
Late arrivals may result in reduced treatment time to avoid impacting subsequent clients.

 

AFTERCARE ADVICE SHEET

Following your treatment, you may experience mild soreness for 24–48 hours.
Recommended advice:
• Drink plenty of water.
• Avoid intense training for 24 hours.
• Apply ice to any tender areas if necessary.
• Light movement and stretching are encouraged.
• Contact your therapist if you experience unusual pain or symptoms.

Red Flag Symptoms (seek medical attention):
• Severe or worsening pain
• Numbness or tingling that does not improve
• Loss of strength
• Dizziness or fainting
